This means that the HTML index of packages (which is generated for  
each help session) cannot be updated - in most cases you get this  
error when the help system was not started yet and thus the index  
doesn't even exist. However, I can only speculate, because you didn't  
tell us which version of R you are using, whether this is the GUI or  
the console, how you installed the packages etc...
