You are right! The whole book is in fact available on-line for reading via Amazon:

Dear Tom,

Many thanks for your nice comments on the book. We realised a few days
ago that the book was out of stock in Europe, but it seems that you can
still get it from the USA (via Springer or Amazon.com). If you order via
through the Springer website (remember the 20% discount) you may need to
adjust the zone to North America to see that the book in still in stock.
Hopefully they will ship more copies to Europe.

As we have mentioned before, any comments on how to improve the book are
welcome (off-list, please).

P.S: By the way, Amazon also allows you to search inside the book
and read a few pages at a time. This is useful even if you have the book
to look for specific keywords!
