[R] r-inferno.pdf with detailed table of contents and bookmarks

That's a reasonable request that I have
planned for whenever I revise it.
However, I'm not going to be doing that
for some time yet (unit of time is somewhere
in the months to years range).

If someone is keen to do that, I can make
the LyX file available to them.
