Use a less unreliable OS?

The copying is done in Perl, and so is out of our control.  This may 
depend on the file system, and I've not seen this (nor have others 
reported it), except on Samba-mounted file systems, about which the FAQ 
says

   Your file system must allow case-honouring long file names (as is likely
   except perhaps for some network-mounted systems).

So sorry, unless that is the explanation this is beyond our control.
